FINGERPRINTING APPOINTMENTS

Step 1
Choose a date and time on the calendar to the right to schedule a fingerprinting appointment. If you're doing this on your phone scroll to the bottom of the screen to see the calendar.

Step 2
Click here to complete the paperwork for the Fingerprinting/Background Check. You must do this prior to your fingerprinting appointment.

You must bring the following to your appointment:

Consent Form
Transaction Receipt
Driver's License

Step 3:
To complete the process you must upload a notarized copy of the Child Maltreatment form to the Department of Human Services. Your background check will not be complete until you do this.
